How to Deactivate your Philhealth Membership

Some of you might be wondering if it is possible to deactivate or cancel your Philhealth membership. If you ever get into a situation wherein your husband or your wife stops working and become unemployed, you can give them the same benefits that the health insurance offer.

What you have to do is to have your husband or your wife deactivate their membership with Philhealth. After the membership has been deactivated or canceled, you then have to enlist them as your dependent. Here’s how.

1. The first thing you have to do is to write a letter addressed to Philhealth requesting a deactivation of your membership. A sample of request of deactivation can be found below.

If you do not want to write your own letter of deactivation, you can go to the nearest Philhealth branch and ask for a deactivation form, which is readily available at the office.

2. Next step would be for the active member to enlist the new dependent. Simply fill out the Philhealth Registration Membership Form. This form can be downloaded here. Once the form had been completely filled out, submit it together with the Letter of Deactivation, 2 Valid IDs, and a copy of Marriage Certificate to the branch. For employees, you can submit the requirements to your HR personnel.

3. After submission, you will be given your Member Data Record (MDR), updated with the new dependent.



It is quite simple. The new dependent can now enjoy the benefits without having to remit contributions. For more information, you can visit the official website.

    1. Your contributions are still with the government agency but if your membership is inactive, you won't be able to avail the benefits when the need arises. If you wish to reactivate your membership, you may do so by visiting the nearest Philhealth Regional Office.

      You can also send your inquiries to actioncenter@philhealth.gov.ph. We are not certain though how long it takes for them to respond.

    2. Hi I'm also a member of philhealth but I can't longer to pay it because I stopped working since Oct. 2020 till now and I am about to giving birth this month feb. 2022. My question is how much can I pay if I am going to pay my missed contribution during that time? Thank you. ☺️

    1. To be eligible to avail of your PhilHealth benefits when hospitalized, the following conditions must be met:

      Payment of at least 3 months’ worth of premiums within the immediate 6 months of confinement. For pregnancies, availment of the newborn care package, dialysis, chemotherapy, radiotherapy and selected surgical procedures, 9 months’ worth of contributions in the last 12 months is needed.
      Confinement in an accredited hospital for 24 hours due to illness or disease requiring hospitalization. Attending physician(s) must also be PhilHealth accredited.
      Claim is within the 45 days allowance for room and board.

      You’ll also need to submit the following documents before being discharged from the hospital for automatic deduction:

      A Clear, Updated copy of your Member Data Record (MDR). If you are dependent, make sure that you are listed in the MDR.
      An original copy of PhilHealth Claim Form 4, which you can get at Philhealth, the hospital or your employer. Submit the original copy signed by your employer.
      Receipt of Premium payments. Employees only need to submit the Certificate of Premium Payments with OR numbers.
      Your PhilHealth ID and a valid ID.

    1. There is no deactivation form that is available for download. You can write your own letter of deactivation or you can go to the nearest Philhealth branch and ask for a deactivation form, which, in reality, is not really a form but more like a letter of consent that you want to deactivate your membership.

    Thank you very much for this very informative blog of yours. I too is looking for the so-called Deactivation form (as per email response of the Philhealth Insurance but can't find any in their website. Reading this article of yours, at least saves me the time of going to the Philhealth office just to get their Deactivation form letter. So, I'll make my own then.

    1. Unfortunately, Philhealth does not refund contributions. If, by mistake, you paid your contributions twice (double contribution), your payment will be carried over to next month. You may visit the nearest PhilHealth Regional Office (PRO) or Local Health Insurance Office (LHIO) in your locality.

    1. Yes. The process of deactivation and adding additional dependents can be done in one day. Also, you are entitled to use the benefits as long as your wife is updated with her contributions.

    1. Yes, it can be accomplished in one day. Simply submit your request for deactivation to PhilHealth.

      To be added as your wife's dependent, your wife must do the following:

      Download PhilHealth Member Registration Form or (PMRF)
      Tick FOR UPDATING on the upper right-hand corner of the PMRF.
      Fill out PMRF as appropriate.
      Submit properly filled out PMRF to the nearest PhilHealth Office.

  15. Hi, I'm recently married and instead of accomplishing the PMRF to update my details I would like to deactivate my account in PhilHealth since I'm currently unemployed, and also, would like to be added in my husband declaration of dependent.

    In writing the Authorization Letter, should I use my current Married name? or should I use my Maiden Name that is the one listed on their system.

    1. You need to first deactivate your Philhealth account. In writing the authorization letter, you should use the name on file so if you are still using your maiden name, you have to use that one.

      Then, your husband can add you as a dependent after deactivation.

Hi can i deactivate my philhealth account kahit kakapagawa ko lang days after and do I have to pay for the remaining utang po between months of february-june meron po kasi akong pinirmahan saying na once I missed paying I have to pay parang interest na 1.25% and gusto ko napo Ideactivate agad because of that.

  17. Pwede po ba yung husband ko nalang po ang magpasa ng deactivation letter ng account ko po? Plan ko po kasi ideactivate account ko para beneficiary na ako ng asawa ko. My concern po is I am a Phil health member but Hindi na updated Ang payments q for 1 yr na po and Ngayon ay gusto q Sana IPA deactivate Yung account q pra maginh dependent aq Ng husband q at mgamit q Yung benefits pg manganak na aq this November. After na mareequest for deactivation yuhg philhealth ng husband ko and adding him as my dependent pwede ko pa ba marequest for refund ang nabayad ko sa hospital for the amount na supposed to be philhealth deductible sa total hospital bill? Upon discharge kasi di na ko nakaabot na maiadd ang husband ko as dependent kaya binayaran ko na lang. Pwede ko pa kaya marecover yun? Salamat sa sasagot.

Hi nagpamember ako noong april 2019 dahil inaapura ako ng amo ko noon pero nung nakakuha na ako ay inalis ako sa trabaho, never pa nahulugan ang philhealth ko hanggang ngayon, 4 years na yung membership ko at wala akong matunong trabaho, paextra extra lang. Pwede ko kaya ipadeactivate nalang yung membership ko
